One other snafu I encountered was Avalon had leveled up to lvl 30, but he was killed by a zombie inflicted status (as only alive member) and when the game continued through game over scene, to restart, he had lost some (all?) of the HP from the previous level gain. i.e. he was level 29 with 2900 hp, leveled to 30, 3000 hp, died, game restart (not reset or load save) and at level 30 still, he had 2900 hp. Was right before magitek facility, the desert snake bone enemy.
I 99% reset/reload when I die anyway, but just happened to notice this.

Yes the timer used for Levy event was coded this way, but it is kinda bad because they are not submerged yet... And a bit messed up towards the player trying to set up the party. Angelo (the event maker from the earlier days) who coded that originally did not code it to allow access to the menu, and for some reason maybe the timer needed or needs to be started from that far before the battle actually starts, when the map loads. I can't recall but can look into it. That event though is one I hate going back to, it's a real mess lol. I guess it can be looked at as a sense of urgency, like the timer start there because that's how long until Levy capsizes the boat, or something. I suppose this would make more sense than drowning anyway since the party still has the underwater breathing helmet to share lol.
Not too sure at all about the Avalon issue. Are you able to reproduce it again easily enough?

What is the song that plays for Ultros, statues battle? Emmm, tasty. Very video-gamey, love it.
(and if you couldn't tell, I am amidst my first play through of RotDS '7' - having played 1.3, 4, 5 and 6 I am slightly inclined to agree with other poster on rotds web, I know the in's and outs of this and, Normal Challenging Hard Insane might be more apt titles for the difficulty - the game never ceases to surprises me no matter what I know and expect. Thanks for the feedback about the difficulty names. I'm still debating this in my head, it's one of the last things to decide on for the 1.7.1 update.

Is it possible you fed Avalon a Golden Apple before dying? That might explain the discrepancy.

Very beginning of WoR I was grabbing fish for Oak and maybe the fifth time I go down, no fish... left the screen to overworld map (didn't fight anything) came back ran around multiple screen changes and at one point when I went into the hut/cabin where Oak is I got a black screen of death. Nothing appeared, but I could still hear the ocean wave sounds. Nothing I could do to undo the black screen. Not sure if this was related to having no fish appear or not.
Luckily I had a save state but, thought it might be worth checking out. Especially if you have just cleared floating continent and watched the 20 minute Zuriel episode without saving in-game or save state that could be rough to do again.
One other note, I did have Snes9x's turbo engaged while screening around trying to make fish appear.
Also, I can't stand the fish 'mini game' takes ages. I never understood its purpose in Vanilla... or now...
Edit: Okay so I have a save state and I think Oak is about to kick the bucket but the game gives me the black screen of death, regardless of any turbo or not.
Really hoping I can find a way past this.
And so far no luck. Resetting in game to the save, still locks. Fought a few battles and visited the mountain, came back, still locks. I guess I may have to save Oak, but I'm worried that even if I do that the 'victory' screen will also lock-up...
Last Edit: and oddly enough 1 fish later after picking up a slightly earlier save state, Oak gives me the raft. So I am past this part anyway.
Only took 25 'in game minutes'

Can't remember the name of the floating medusa head enemy on floating continent, but this enemy as well as Undying enemy around Stardust Village region (WoR) both have the auto-life 'sound' play as if they are coming back to life and then, nothing, end of battle. Likewise if medusa head casts auto-life on a skeleton enemy in its formation, kill the skele, sound plays, but skele doesn't come back to life.
Not sure if intentional.

Now, to figure out what caused the Life 3 to monsters to break and/or no longer function. I can confirm it on my side here as well. Going to get on this now, got lots and lots of back ups it's all just a matter of narrowing it down.

EDIT: seems it's not working for all difficulties actually. WEIRD! It's something to do with the monster stats not picking up or breaking life 3 status, for some reason. A ROM I have 5 days before this issue is present is working...
